Parent Handbook Acknowledgment — Discovery Pathways Academy 2026

I acknowledge that I have received and reviewed the Discovery Pathways Academy Parent Handbook and fully understand the policies, procedures, and expectations outlined herein.

I agree to abide by all program guidelines and to support a safe, respectful environment for all children and staff, including but not limited to:

• Arrival and dismissal procedures, including daily sign-in and timely pickup
• Health and illness policies — I will keep my child home when ill
• Medication policy — all medications will be in original containers with written authorization
• Behavior expectations and discipline policy
• Electronics policy — personal devices restricted during camp hours
• Food and allergy disclosures as required
• Communication responsibilities — maintaining updated contact information

I understand that repeated unsafe or disruptive behavior may result in a parent conference or dismissal from the program.

Media & Photo Release — Discovery Pathways Academy 2026

I, the undersigned parent/legal guardian, hereby grant Woman Rediscovered and Discovery Pathways Academy permission to photograph and/or record video of my child during camp activities, events, and programming.

Photos and videos may be used for:

• Camp promotion materials and printed publications
• Social media platforms managed by Woman Rediscovered / DPA
• Program newsletters and ministry publications
• Website content and digital marketing

No monetary compensation will be provided. I retain the right to request removal of any specific image by submitting a written request to program administration. All media will be used in a respectful manner consistent with the values of Woman Rediscovered.

Parents who wish to opt out must submit a written request to program administration.

Medical Authorization — Discovery Pathways Academy 2026

I, the undersigned parent/legal guardian, hereby authorize Discovery Pathways Academy staff and any on-site medical personnel to provide emergency medical care and first aid to my child if I cannot be reached in a timely manner.

This authorization includes:

• Administration of first aid for minor injuries (cuts, scrapes, headaches, etc.)
• Administration of prescription or over-the-counter medications as documented in my child's health file, in original labeled containers per written authorization
• Contacting emergency services (911) and authorizing emergency transport by ambulance if deemed necessary
• Communicating with emergency contacts and healthcare providers on my behalf

I understand that in the event of serious illness or injury, DPA will: contact emergency services if necessary, notify parents/guardians immediately, and act in the best interest of the child until parent arrival.

I confirm that emergency medications such as EpiPens and inhalers have been disclosed and will be provided during enrollment. I have disclosed all known medical conditions, allergies, and medications in the Camper Information section.

Liability Waiver — Discovery Pathways Academy 2026

I understand and acknowledge that participation in the Discovery Pathways Academy Summer Day Camp involves physical activities, outdoor recreation, and field excursions that carry inherent risk of injury.

By enrolling my child, I voluntarily agree that:

• I accept the inherent risks associated with camp activities including outdoor games, sports, nature activities, field trips, and transportation
• I release and discharge Woman Rediscovered, Discovery Pathways Academy, Fairfield Township Parks, their staff, volunteers, and agents from liability for injuries, losses, or damages arising from my child's participation, except in cases of gross negligence or willful misconduct
• I agree to be responsible for medical expenses incurred by my child not covered by insurance
• I confirm all relevant health information has been disclosed and my child is physically able to participate in camp activities

I acknowledge that DPA maintains active supervision of children at all times including indoor activities, outdoor recreation, field trips, transportation, and restroom transitions, and conducts frequent attendance checks throughout each day.

This waiver is governed by the laws of the State of Ohio.

Code of Conduct — Discovery Pathways Academy 2026

Discovery Pathways Academy is a faith-based program rooted in respect, integrity, and Christian values. All campers are expected to uphold a high standard of conduct that supports a positive and safe community for all participants.

Campers are expected to:

• Respect staff, peers, and all adults at all times
• Follow all safety instructions promptly and cooperatively
• Use appropriate language in all settings
• Participate cooperatively in all activities including faith-based programming
• Respect program property, facilities, and others' belongings
• Remain in designated areas at all times

Prohibited items & behaviors: Personal cell phones during camp hours, weapons of any kind, tobacco, alcohol, or illegal substances. Physical fighting, bullying, or verbal abuse of any kind will not be tolerated.

DPA uses positive guidance and redirection. Physical punishment, humiliation, verbal abuse, and harsh or degrading discipline are strictly prohibited by our staff. Repeated unsafe or disruptive behavior may result in a parent conference or dismissal from the program without refund.

I confirm I have reviewed these expectations with my child and they understand and agree to abide by the DPA Code of Conduct.

Transportation Authorization — Discovery Pathways Academy 2026

I authorize Discovery Pathways Academy transportation staff to transport my child to and from the designated pickup location for the 2026 Summer Day Camp.

Per the DPA Transportation Policy, I understand and agree that:

• My child must follow all transportation safety rules at all times
• Seat belts must be worn at all times when applicable
• Staff will maintain attendance verification during loading and unloading
• Transportation fees apply separately and will be communicated by administration after enrollment approval
• Pickup and drop-off times will be confirmed by administration prior to camp start
• My child must be at the designated pickup location on time; late arrivals may be skipped without refund for that day
• A responsible adult must be present at the drop-off location to receive my child
• Transportation routes may change with reasonable notice from administration

Transportation areas served include: Hamilton, West Chester, Liberty Township, Monroe, Springdale, Trenton, Colerain, and select additional locations. Contact administration at 513-233-5878 for areas not listed.

Parents must complete this transportation authorization form prior to participation in any transportation services, per DPA policy.

Water Play & Outdoor Activity Consent — Discovery Pathways Academy 2026

Discovery Pathways Academy summer programming includes supervised outdoor water play activities as part of our enrichment curriculum. Water play activities may include sprinkler play, water games, slip-and-slide, and similar supervised activities appropriate to each child's age group.

By signing this consent, I authorize my child to participate in water play and outdoor recreation activities, and I acknowledge the following:

• All water activities are supervised by trained DPA staff at all times
• Activities are age-appropriate and designed with child safety as the top priority
• My child will be required to follow all staff instructions during water activities
• I will send my child with appropriate clothing/swimwear and a towel on designated water play days (advance notice will be provided)
• I understand that outdoor activities may also include sun exposure, and I authorize staff to apply sunscreen provided by the program as needed
• I will disclose any skin conditions or sensitivities that may be affected by water or sunscreen
